Infectious complications after transrectal MRI-targeted and systematic prostate biopsy
Purpose To compare infectious complications after transrectal systematic prostate biopsy (SB) and magnetic resonance imaging (MRI)-targeted biopsy (TB) in a large retrospective cohort to assess whether one technique is superior to the other regarding infectious complications. Methods A total of 4497 patients underwent 5288 biopsies, 2875 (54%) SB and 2413 (46%) MRI-TB only. On average, 12 SB cores and 3.7 MRI-TB cores were taken per biopsy session during the study period. Infection-related complications within 30 days were compared. The primary endpoint was a positive urine culture. Secondary endpoints were positive blood cultures, urine tests with elevated leukocytes ≥ 100 E6/L and elevated C-reactive protein (CRP) ≥ 100 mg/L. Chi-square test was used to compare the cohorts. Results Positive urine cultures were found in 77 (2.7%) after SB and in 42 (1.7%) after MRI-TB ( p  = 0.022). In total, 46 (0.9%) blood culture positive infections were found, 23 (0.9%) occurred after SB and 23 (1.0%) after MRI-TB, ( p  = 0.848). Urine tests with elevated leukocytes ≥ 100 E6/L were found in 111 (3.9%) after SB and in 61 (2.5%) after MRI-TB ( p  = 0.006). Elevated CRP ≥ 100 mg/L was found in 122 (4.2%) after SB and in 72 (3.0%) after MRI-TB ( p  = 0.015). Blood cultures were drawn more often after SB than after MRI-TB, but the difference was not statistically significant. However, urine cultures and CRP were taken more often after SB than MRI-TB. Conclusion Blood culture positive infections were equally rare after SB and MRI-TB. However, all other infectious complications were more common after SB than MRI-TB.
Barriers, Springboards and Benchmarks: China Conceptualizes the Pacific “Island Chains”
US government reports describe Chinese-conceived “island chains” in the Western Pacific as narrow demarcations for Chinese “counter-intervention” operations to defeat US and allied forces in altercations over contested territorial claims. The sparse scholarship available does little to contest this excessively myopic assertion. Yet, further examination reveals meaningful differences that can greatly enhance an understanding of Chinese views of the “island chains” concept, and with it important aspects of China's efforts to develop as a maritime power. Long before China had a navy or naval strategists worthy of the name, the concept had originated and been developed for decades by previous great powers vying for Asia-Pacific influence. Today, China's own authoritative interpretations are flexible, nuanced and multifaceted – befitting the multiple and sometimes contradictory factors with which Beijing must contend in managing its meteoric maritime rise. Grading Evolution and Contemporary Prognostic Biomarkers of Clinically Significant Prostate Cancer
Gleason grading remains the strongest prognostic parameter in localized prostate adenocarcinoma. We have here outlined the evolution and contemporary practices in pathological evaluation of prostate tissue samples for Gleason score and Grade group. The state of more observer-independent grading methods with the aid of artificial intelligence is also reviewed. Additionally, we conducted a systematic review of biomarkers that hold promise in adding independent prognostic or predictive value on top of clinical parameters, Grade group and PSA. We especially focused on hard end points during the follow-up, i.e., occurrence of metastasis, disease-specific mortality and overall mortality. In peripheral blood, biopsy-detected prostate cancer or in surgical specimens, we can conclude that there are more than sixty biomarkers that have been shown to have independent prognostic significance when adjusted to conventional risk assessment or grouping. Our search brought up some known putative markers and panels, as expected. Also, the synthesis in the systematic review indicated markers that ought to be further studied as part of prospective trials and in well characterized patient cohorts in order to increase the resolution of the current clinico-pathological prognostic factors.
The Challenge of Maintaining Stormwater Control Measures: A Synthesis of Recent Research and Practitioner Experience
The methods for properly executing inspection and maintenance of stormwater control measures are often ambiguous and inconsistently applied. This paper presents specific guidelines for inspecting and maintaining stormwater practices involving media filtration, infiltration, ponds, and permeable pavements because these tend to be widely implemented and often unsatisfactorily maintained. Guidelines and examples are based on recent scientific research and practitioner experience. Of special note are new assessment and maintenance methods, such as testing enhanced filtration media that targets dissolved constituents, maintaining proper vegetation coverage in infiltration practices, assessing phosphorus release from pond sediments, and the development of compressed impermeable regions in permeable pavements and their implications for runoff. Inspection and maintenance examples provided in this paper are drawn from practical examples in Northern Midwest USA, but most of the maintenance recommendations do not depend on regional characteristics, and guidance from around the world has been reviewed and cited herein.
Phosphate Removal from Agricultural Tile Drainage with Iron Enhanced Sand
Can iron enhanced sand filtration capture total phosphorus and soluble phosphorus (phosphate) from agricultural tile drainage? A monitoring study measured the total phosphorus and phosphate capture performance of an iron enhanced sand filter (IESF) installed to treat agricultural tile drainage in Wright County, MT, USA. Overall, for natural rainfall-induced tile drainage events monitored between June and November 2015 and again in 2016, the IESF captured 66% ± 7% (α = 0.05, n = 21) of the influent total phosphorus mass and 64% ± 8% (α = 0.05, n = 31) of the influent phosphate mass. Removal of total phosphorus and phosphate was approximately uniform for large and small rainfall-induced tile drainage events and varied from 42% to 95% for total phosphorus and 9% to 87% for phosphate. The IESF treated 290 m of treated depth since installation, and results indicate that performance is similar or better than constructed wetlands or other IESFs, though not as good as laboratory experiments of IESFs. Routine and non-routine maintenance was performed throughout the project to ensure adequate phosphorus capture and flow rate through the IESF.
